(2) You provide us with the following information:

(i) Name, (ii) Affiliation, (iii) Position, (iv) whether or not you prefer lunch being provided; and (v) whether or not you plan to participate in the conference reception dinner (around 19:00). For items (iv) and (v), you just answer either ``YES'' or ``NO.'' Here is an example:

When you come to the conference, we ask you to pay the registration fee for 1,000 (JPY) at the registration desk. This is the only cost you have to pay for the conference so that all the costs for lunch, reception, coffee and etc. are included in this fee. Your contribution on this is very much appreciated.
